Rakanishu always spawns next to the Cairn Stones that are used to activate the portal to Tristram during the Search for Cain quest.
When attacking Rakanishu, melee heroes need to be careful because his Lightning Enchanted status causes several Charged Bolts to radiate out from him each time he is damaged. Very quick attacks on Rakanishu will result in an equally quick death if you do not watch yourself carefully, especially on the higher difficulty levels. Rakanishu is the first of the Lightning Enchanted bosses in Diablo II, but as the game progresses they become more numerous. High Lightning resist is often a must in the higher difficulty levels to deal with these monsters.

Fallen monsters (Fallen, Carvers, Devilkin, Dark Ones and Warped Ones) are sometimes heard saying "Rakanishu" in the game.
